WHAT TO EXPECT

Your first visit with us is a chance for us to get to know each other. 

When you first arrive there will be a small amount of paperwork to complete.  We will then start our session with a conversation about you: any concerns you have, your history, what have you tried in the past and your goals.
This will be followed by a simple assessment based on what you are hoping to achieve and finally we will figure out a plan together for going forward.

FUNDING

Chronic Condition Management (Medicare)

A Chronic Condition Management (CCMP) Plan is available when a 'chronic medical condition has been, or are likely to be, present for at least 6 months'.  It allows for up to 5 Medicare subsidised allied health sessions per calendar year. 

 

You'll need a GP referral to access this plan. An out-of-pocket fee applies for each session

Please Note: You will be required to pay the full amount on the day and then we will refund the rebate directly back to your bank card.  

Department of Veteran Affairs (DVA)

White and gold card holders are entitled to treatment for clinically necessary conditions.  

To access this treatment you will need an appropriate referral from your GP.

T2 Diabetes Group
(Medicare)

8 Medicare subsidised sessions are available for people 'assessed as suitable for a type 2 diabetes group service'. 

 

To access these sessions you will need a GP referral and there will be a $20 out of pocket fee for the initial appointment and a $10 out of pocket fee for each group session.

WorkCover WA & MVA

Exercised based programs are available to those who have been approved by their insurer. 

To access this service you will need a referral from a GP, other appropriate healthcare provider or insurer. 

Private Health Insurance

Private health insurance rebates are available for most services and they vary according to your individual level of cover.  It is best to call your provider to find out what you are covered for.

NDIS

We are not registered with NDIS however we are still able to provide services for those who are self-managed and plan-managed provided you have the appropriate funding.  

 

You will need to know how your funds have been allocated, this can be done by contacting your plan manager or NDIS directly.

CANCELLATION / NON-ATTENDANCE POLICY

We are a busy clinic with clients often wishing to get earlier appointments so if for any reason you are unable to attend your appointment we ask that you please show us the kindness and respect of notifying us as soon as possible so we have the opportunity to fill the space. 

We understand that life can be unpredictable and so we strive to be as accommodating as we can.

In order to be fair to our practitioners, the business and all our clients we have decided that missed appointments or cancellations with less that 24 hours notice will result in a $50 cancellation fee. 

 

Please note: Third parties (Medicare, DVA, Workcover) will not fund this cancellation fee.
